Properties: Isosoibide Dinitrate is a vasodilator with general properties similar to those of glyceryl trinitrate. It is an active metabolite of the Isosorbide Dinitrate. Indications: Iso-Dinitrat is used for the prophylaxis and treatment of coronary insufficiency disease (angina and myocardial infarction) and as adjunct in the management of congestive heart failure. Contra-indications: Iso-Dinitrat in contra-indicated in patients who have shown hypersensitivity to the drug or to other nitrates or nitrites and patients with severe hypotension, hypovolaemia, marked anemia, constructive pericarditis or raised intracranial pressure due to head trauma or cerebral hemorrhage. Side-effects: The majority of side-effects are mild, temporary, and rarely lead to discontinuation of the drug. Such side-effects include headache, hypotension and related feeling of nausea, vomiting, weakness and pallor, cutaneous vasodilatation and flushing. Precautions: Iso-Dinitrat should be used with caution in: - Patients with severe impairment of renal or hepatic function, hypothyroidism, malnutrition, hypothermia, and glaucoma. - Patients using diuretics. - Patients suffering from hypertrophic cardiopathy as it may aggravate pectoris symptoms. - Care should be taken in patients with early myocardial infarction. Parameters should be monitored in order to avoid the risk of sudden hypotension associated with early administration of organic nitrates, blood pressure, and other hemodynamics. Drug Interactions: Alcohol enhances the hypotensive effects of Iso-Dinitrat. Calcium channel blockers when used with Iso-Dinitrat may cause marked symptomatic orthostatic hypotension, therefore dose adjustment of either drug may be necessary. Dosage and Administration: Angina pectoris: 30-120 mg daily in divided doses according to the patient's needs. Dosage increment should be gradual to avoid side-effects up to 240 mg daily in divided doses may be necessary. Heart failure: 30-160 mg daily in divided doses up to 240 mg daily may be required. |
